4. Sign Volunteer Handbook

Sign the Volunteer Handbook which will be emailed to you in 3-5 days. You may read the handbook (but not sign it) beforehand using the button below.

5. Sign up for your first route.

A member of our team will contact you once your application and background check have finished processing to guide you in setting up your first route.

I hereby affirm that I have a current and valid (unexpired) California Drivers’ License and will maintain such so long as I participate as a driver performing Meals on Wheels Connect activities.

As a driver for Meals on Wheels Upland, I hereby agree that I will maintain automobile bodily injury and property damage liability coverage in the amount of at least $50,000 per accident on any motor vehicle used by me while participating in Meal on Wheels Upland activities.

Practices and Procedures:

I understand that I will be trained by Meals on Wheels Connect. I will comply with all training procedures and will always practice safety and report any hazards which I observe. I will also make every effort to obey safety precautions provided in writing and/or explained to me.

Volunteer Waiver and Release:

I hereby agree to release Connect Senior Services and its Upland Meals on Wheels program from any and all liability relating to injuries that may occur during my volunteer activities. I acknowledge the risks involved in Meals on Wheels Connect activities, in which I am participating voluntarily. I also fully understand that Connect Senior Services does not assume responsibility for, or any obligation to provide, financial or other assistance, including but not limited to medical, health or disability insurance, for any injury, illness, death or property damage sustained by me while participating in Connect Senior Services activities. I hereby release and forever discharge and hold harmless Connect Senior Services and its successors, assigns, directors, officers, agents, members, and volunteers from any and all liability claims and demands of whatever kind of nature, either at law or in equity, which arise while I am performing my volunteer activities with Connect Senior Services.

Photo Release

I hereby grant and assign Connect Senior Services all right, title, and interest in all photographic images and video or audio recordings that are made during my volunteer time with Connect Senior Services.
